The staff of Grandview Elementary is excited to partner with you on the beginning of your child’s journey into elementary education. Below is a series of checklists of items needed in order to complete your child's registration.

• Emergency Treatment Release Form
• Residency Verification Form
• Publication Consent Form
• Home Language Survey
• Household & Income Form
  • one form needed per household, per year
• GES Walking Field Trip Consent Form
• GES Student Medical Information Form
• GES Note From Nurse 
• GES Bus Transportation Form (for Preschool-Kindergarten students ONLY)

MEDICAL INFORMATION FORMS  (to be completed by healthcare providers)

• Kentucky Immunization Form
• Preventative Health Care Examination Form
• Kentucky Eye Exam Form for School Entry
• Kentucky Dental Screening/Examination Form for School Entry

 ADDITIONAL ENROLLMENT ITEMS  (needed to verify ID and proof of residency)

• Copy of Child's Social Security Card
• Copy of Child's Birth Certificate
• Proof of Residency (examples: copy of lease, deed, or current utility bill)
